All patients need to fill out New Patient Form, Patient Health Questionnaire, and sign the HIPPA form at the bottom. If you are bringing a child under 18 years old, please fill out and sign the Minor Child Consent Form. You may either print them out and complete them by hand or complete them online and print them to bring with you to your appointment.
